WATERLOO - Workers who want to recognize their employers for a job well done will get their chance through a new Employers of Choice Award program, sponsored by Courier Communications.

Employers who qualify for the recognition meet the following criteria:

Cares about quality-of-life issues.

Supports family and allows flexibility for family issues and events.

Provides opportunities to learn new skills.

Provides employees with regular feedback from a top management team.

Recognizes employees for a job well done.

To nominate your company, complete a form with the criteria listed below and attach a description in no more than 200 words of why your nomination meets the criteria.

Nomination forms also are available on the Courier's Web site, www.wcfcourier.com.

On each form, list the following:

Company name.

Company president/chief executive officer's name.

Company telephone number.

FAX number.

Business address.

City, state, ZIP code.

E-mail address.

Name of employee submitting nomination.

Company name.

Employee telephone number.

Employee address, with city, state, ZIP code.

Complete the form and mail to Employers of Choice, c/o Marketing, P.O. Box 540, Waterloo, IA 50704 or fax to Attn: Employers of Choice at (319) 234-3297.

If you prefer to fax or mail your entry, print your responses directly on the form. Your essay may be included separately. Be sure to include your address and phone number in the event more information is required. All information will be confidential.
